Before you begin the online registration process, please read the above directions gather the following documents, and scan or take a picture of them with your device so you can upload them during the online registration process. 

1. Proof of date of birth: Please provide your child’s original birth certificate. If your child’s birth certificate is unavailable, one of the following can be used for identification: passport or certified record of baptism. 
2. Proof of residency - Chart
3. Parent/Guardian Photo Identification
4. Official Student Immunization Record, and most recent physical 
5. Custody Papers (if applicable). Must be a signed copy.

Screening for the Registered Children will be held in May. You will be contacted to schedule an appointment time.
